Write one question per row
The question in the question column. Each answer in an answer column and, next to it in correct, true if it is right or false if not.
true or false (the sheet may show it as TRUE: that works too)
Two answers at least, ten at most
Several true = a multiple-answer question
The explanation is optional
True or false: two answers, “True” and “False”, and mark the right one with true.
More than four answers? Add the columns answer5 and correct5, and so on up to answer10.

Download it as CSV
Save it as CSV, comma-separated, in UTF-8. Pick your program:
Google Sheets
File → Download → Comma-separated values (.csv). The file goes to Downloads.
Excel
File → Save As → CSV UTF-8 (Comma delimited) (*.csv). If Excel saves it with semicolons, as it does in Spain and Brazil, that is fine too: the import reads both separators.
Numbers
File → Export To → CSV… Under Text Encoding pick Unicode (UTF-8).
On a phone
In the Google Sheets app: ⋮ → Share & export → Save as → CSV. If you see no CSV, open the sheet on a computer, or copy the rows and paste them into the box in step 4.

Pick the file or paste its text. You will see your questions as the app will show them, or the exact line to fix.
